Fill out a confusing official form correctly the first time
Walks you through a confusing form section by section, explains each question, and flags the mistakes that cause delays or rejections.
The prompt
Help me fill out an official form correctly.
Form name and who issued it: [e.g. "benefit application from my state agency", "insurance claim form", "school enrollment form"]
Form questions or sections: [PASTE THE TEXT, or describe the sections I'm stuck on]
My situation: [the facts the form is asking about - leave out ID numbers and passwords]
What confuses me: [specific questions or terms]
Deadline and how I'll submit it: [online / mail / in person, and by when]
Steps:
1. Explain in plain words what each section is really asking and why.
2. For each question I'm unsure about, tell me what kind of answer fits my situation, and where on my own documents I'd find it. Don't fill in facts I haven't given you.
3. Flag common mistakes on forms like this: blank required fields, mismatched names or dates across documents, missing signatures or dates, wrong version of the form, unchecked boxes, missing attachments.
4. Point out any question where a wrong answer could have serious consequences (e.g. a statement signed under penalty of perjury) and tell me to confirm it with the office that issued the form.
Return:
- A section-by-section guide: Section | What it asks | What I need | Watch out for.
- A list of documents and attachments to gather.
- A final check-before-I-submit list.
- Questions to ask the issuing office, if anything is still unclear.
Never invent rules, deadlines, or eligibility criteria - mark them [CONFIRM WITH THE OFFICE]. Remind me not to paste full ID numbers or account numbers into the chat.
Highlighted [brackets] are blanks to replace with your own details.
