Extract decisions, actions, risks, and deadlines from a document
Turns any document or long update into a clear list of decisions made, actions with owners, risks, deadlines, and open questions.
The prompt
Go through the material below and pull out everything I need to track.
Material: [PASTE document, report, long email thread, or project update]
Context: [e.g. "steering committee pack for the CRM migration"]
Today's date: [DATE, so you can flag what's overdue]
Return five tables:
1. Decisions made: | Decision | Who made it | Date | Section or quote |
2. Actions: | Action | Owner | Due date | Status if stated | Section |
3. Risks and issues: | Risk or issue | Likely impact | Mitigation mentioned | Owner |
4. Deadlines and milestones: | Date | What is due | Who | Overdue or at risk? |
5. Open questions: | Question | Why it matters | Who could answer it |
Then:
- Top 3 things to follow up on this week.
- Gaps - actions with no owner, deadlines with no date, decisions that are implied but never confirmed.
Rules:
- Only list what the material says or clearly implies; mark implied items as (implied).
- Never invent owners or dates - write [NO OWNER] or [NO DATE].
- Keep each row to one line so I can paste the tables into Excel, a tracker, or a Teams or Slack message.
Highlighted [brackets] are blanks to replace with your own details.

How this prompt is built
This prompt follows the RTF structure (Role, Task, Format). RTF tells the model who to be, what to do and what shape the answer should take. It is the quickest structure that still gives noticeably better results than a bare request.
